For this consultation, you will be asked to submit available medical records, medications, herbal regimens, diagnostics including labs, and radiology reports for evaluation. You will be asked to complete a wellness, activity, and dietary summary. Upon completion, you will be scheduled for a 60 minute telecommunication consultation at which time we will discuss intentional lifestyle changes to create individualized health goals and specific behavioral changes necessary to achieve them.

After the virtual meeting, you will receive a summary emailed to you on the identified focus areas for health promotion.
